To apply for membership, please complete and submit the form on this page. You will receive a confirmation email with information on where to send your check payment.
Please print and complete the attached form above working from yourself (as Generation 1) to Edward Howell (1584-1655), including as much information as possible. Names should be maiden names where applicable, locations should include city/town, county, and state. Proof can be a copy of a certificate, a church, probate, or land record, an approved lineage society application (DAR, SAR, etc.), published genealogy, or other form of documentation. Acceptability of proof will be determined by the Edward Howell Family Association Genealogist. If more space is needed, please use a separate sheet, referencing the appropriate Generation number. Cross out or add generations as needed.
